The 123/TUNE+ 4-R-V-EXT-BLUE is specially designed for the VW bus, where the engine is located at the back, under a metal cover. This creates a kind of Faraday cage; electromagnetic radiation is blocked (both ways). What this means for the TUNE+ is that the Bluetooth connection falters at best, and does not work at worst. The 123/TUNE+ 4-R-V-EXT-BLUE completely eliminates this problem; the external Bluetooth module is mounted outside the engine compartment, to provide an always reliable Bluetooth connection.
